In case if any parent(s) are not alive, then death certificate copy is required with the documents copy of guardian (income certificate, affidavit etc). No self-Income Certificate.

Photocopy of Bank Passbook – Facility for SC/ST students (Indian Bank, University Campus)

High School Tracker (From Scholarship Portal)

 Active NPCI Printout Slip

 PAN Card Copy (Below 18 years can also apply for the pan card, pan card issue without photograph card.)

SCHOLARSHIP FORM

To fill out a scholarship form, follow the specific guidelines provided by the scholarship program, ensuring you complete all required sections accurately and submit necessary       documents.

Step-by-Step Guide to Filling Out a Scholarship Form

1. Register on the Scholarship Portal:

Before applying, you may need to register on the scholarship portal. For example, for the UP Scholarship, complete the OneTime Registration (OTR) process first. This            involves providing your personal details, such as your name, date of birth, and caste        category. 

1. Gather Required Documents:

Prepare the necessary documents that may include:

• Class X- and XII-mark sheets
• Aadhaar card
• Income certificate
• Any other documents specified by the scholarship provider. 

1. Complete the Application Form:

• Log in to the scholarship portal using your registered email and password.
• Fill in the application form carefully, ensuring all information is accurate. Common  fields include personal details, educational background, and financial information. 
• Pay attention to any specific instructions regarding the format of the information 

(e.g., date formats, file sizes for uploads).

1. Review Your Application:

Before submitting, review all entries for accuracy and completeness. Check for any 

errors or missing information that could lead to disqualification.

1. Submit the Application:

Once you are satisfied with your application, submit it through the portal and after that submit the hardcopy of your scholarship form to the college along with necessary documents. Make sure to note any confirmation messages or reference numbers provided after           submission.

1. Follow Up:

After submission, keep track of your application status through the scholarship portal.    Some programs may require additional steps or interviews, so stay informed about any 

updates.
